Request In-person:
8:00 a.m. to 4:30 p.m., Monday - Friday at Village Hall located at 118 W. Cook Avenue. (*If you require alternate hours for report pick-up, call the Records Department. Copies of accident reports are $5.00 each (cash, credit, and check are acceptable forms of payment). A FOIA (Freedom of Information Act) request will need to be filled out prior to obtaining a report in-person.Request Online:
Accident reports are accessible 24 hours a day. All convenience fees are added to the cost of the report and are directly credited to BUYCRASH. (Must have report # from Police Officer to obtain report)Request by mail:
Accident reports may be requested by mail by sending a self-addressed, stamped envelope to the Records Division. Requests should include the date, time, and name of driver(s) involved in the accident or a report number, along with cash or check for each report requested. Requests should be mailed to Libertyville Police, Attn: Records Department, 118 W. Cook Ave., Libertyville, IL 60048.PLEASE NOTE: Reports typically require two (2) business days to complete. Please wait at least 48 hours before submitting a duplicate request.
